Live-in home care: what it costs, and how hiring one actually works
Live-in home care means one caregiver lives in the home and covers the day and night in one arrangement — the alternative to stacking hourly shifts or moving a parent into a facility. This page uses real numbers: Only Livein tracks live caregiver postings every 8 hours, counted October 21, 2026 at 45 current listings.
What live-in care costs
The caregiver's pay is the core cost. In current listings, caregiver day rates cluster around $200–$350/day with a US median near $245/day — roughly $7,350 a month at the median for continuous coverage, before agency fees if you use one. Cases at $400/day and above exist but are the premium end (complex care, short notice, rotations).
Why families choose live-in over 24-hour hourly care
"24-hour care" billed hourly means two or three shifts a day. At $30/hour, round-the-clock coverage is about $720/day — more than double a typical live-in day rate, with a stranger rotation through the house. A live-in caregiver is one consistent person who knows the routines, the medications, and the house. That consistency is the real product; the savings are a bonus.
Agency or hire directly?
- Through an agency: the agency screens, employs, insures, and — importantly — sends a replacement when your caregiver is sick or quits. You pay for that margin: the family rate is higher than what the caregiver receives.
- Directly (private family hire): you pay the caregiver's full rate and keep the difference, but you are the employer — payroll, taxes, backup coverage, and the hiring risk are yours. In most states a live-in caregiver is a household employee (W-2), not a contractor.
Plenty of families start with an agency for the first placement and hire directly later. Our post-a-case page is free for families: cases paying $400+/day are emailed to our signed-in members the moment they are confirmed.
What good looks like
- Pay stated plainly, in writing, before the start date — day rate, days off, and night expectations.
- A private room and uninterrupted sleep time in normal cases.
- A named backup plan for days off and emergencies.
- No caregiver should ever be asked to pay a fee, buy equipment, or cash a check to start — those are scam markers.
